Membership Application Form
Below is a link to a PDF file which can be downloaded and filled with your information and emailed to us.
Ideally your device should use Adobe Acrobat Reader to open the file. The file will also work in most browsers but to use the fillable facility, you may need to open the file in a new tab (see Fillable Form Instructions).

Using the form
When the Membership Application Form file is opened there are various fields indicated in blue. Click on the first field (First name) and then you can key-in the information. You can use the Tab button to move to the next fields - or use your mouse pointer.
When complete, SAVE the file naming it your SURNAME.FIRSTNAME and then email the document as an attachment to: mgclement48@gmail.com
