DIGITAL version - to be filled with your information digitally and emailed to us
Using the digital form
Ideally your device should use Adobe Acrobat Reader to open the file. The file will also work in most browsers but to use the fillable facility, you may need to open it in a new tab.
You can get Adobe Acrobat Reader HERE free to install on your computer or device.
When the digital Form is opened there are various fields indicated in blue. Click on the first field (First Name) and then you can key-in the information. You can use the Tab button to move to the next fields - or use your mouse pointer.
When complete, SAVE the file naming it your SURNAME.FIRSTNAME and then email the document as an attachment to: mgclement48@gmail.com

Renewing members and Gift Aid
For RENEWING members only for 2026/7, you will have received an email request to renew. You were also invited to grant GIFT AID and links to the Opt-In form were detailed in the email.
